Despite the complexity of current internal medicine texts, there's still a need for a concise resource for students and busy practitioners.

While recognizing the importance of pathophysiology, we emphasize practicality in identifying the three most likely differential diagnoses. In this book, we provide lists of both common and uncommon causes of medical problems, focusing on alphabetical order rather than prevalence. The edition maintains sections on presenting complaints, physical findings, laboratory abnormalities, and introduces a new section on imaging patterns. We cover diseases of major organ systems and expand authorship to include expertise in cardiology and information on systemic manifestations of behavioral, dermatological, and ophthalmological problems.

Rejecting a purely algorithmic approach, we highlight key clinical clues to guide clinicians, combining history-taking, physical examination, laboratory tests, and imaging findings. The complexity of internal medicine requires acknowledging that not every case follows textbook patterns. We stress the importance of thorough history-taking and clinical examination, encouraging clinicians to revisit the basics when faced with complicated cases. The process involves continuous questioning and investigation without hesitation to start afresh when necessary.
